How do I create an invoice for free?

What should a freelancer's invoice include?
A professional invoice includes your business name and contact details, your client's details, a unique invoice number, the issue and due dates, an itemized list of the work (description, quantity, rate, and amount), the subtotal, any tax or discount, and the total due. Can I add my own logo?
